The Continuum of Care (CoC) is a shared community effort to help people experiencing homelessness find safe, stable housing and the support they need to rebuild their lives. It focuses on meeting each person where they are, walking alongside them as they move toward independence, and putting steps in place to not only end homelessness but help ensure it doesn’t happen again.

What is a continuum of care?
A Continuum of Care (CoC) is a group of local organizations, agencies, and community partners working together to help people experiencing homelessness find stable housing and rebuild their lives.
Together, they coordinate everything from outreach and emergency shelter to transitional and permanent housing, along with the support services people need along the way.
Because every situation is different—whether it involves job loss, domestic violence, mental health challenges, or other hardships—the CoC brings the community together to respond in a thoughtful, connected way. The goal is simple: help people find stability and make sure they don’t end up back in homelessness again.
This work is supported through funding and partnerships at the local, state, and national levels—helping connect people not just to housing, but to the broader resources they need to move forward.
Community partners
We work alongside a wide network of local and regional partners who help make this work possible, including housing authorities, city leaders, behavioral health providers, shelters, legal aid organizations, faith communities, schools, and nonprofits such as United Way and Habitat for Humanity.
We also collaborate with workforce and employment services, free clinics, community action agencies, landlords, and civic groups like Lions Clubs, Rotary Clubs, and Shriners—along with many other community organizations committed to helping people move toward stability.
